-continue doing my task regarding 360 feedback survey. To transfer the data into the power bi because of new entry.

-Started the day with Morning meeting at control room 8:30 am. There one safety and HSE moment which is regarding Learning from the Worst Ammonia Accident. All Plant are shutdown currently due to some technical issues.
-continue doing my task regarding 360 feedback survey. To transfer the data into the power bi because of new entry.

-Started the day with Morning meeting at control room 8:30 am. There one safety and HSE moment which is regarding Avoiding Car Fire. Theres several tips and action are shared while handling the situation. Part of it was do not panic, change to geap P, and exit the vehicle. All Plant are shutdown currently due to some technical issues.
-continue doing my task regarding 360 feedback survey. To transfer the data into the power bi because of new entry and improvise it to the most efficient way.

-Hazzad and Hunt at Butanol Plant. Inspect and observe any potential hazzard around the plant area. Butanol also doing some wet test to one of their system.

-Started the day with Morning meeting at control room 8:30 am. There 1 safety and HSE moment. Some mistakes take time to become incidents.
-
We are human; we make mistakes. Even highly trained people like pilots and astronauts make mistakes!
-
There are two kinds of mistakes:
-
Not taking the required action or doing it at the wrong time (error of omission).
-
Knowing the correct thing to do but choosing to do something different (error of commission).
-
-
Safety barriers prevent process safety incidents. It may take time for all the barriers to fail, then an incident occurs.
-
However, before then, there may be warning signs of failed barriers – noises, odors, drips, etc. Watching for and responding to those warning signs is how to prevent the major incidents.
