If you notice equipment that looks similar but operates differently, tell your supervisor. There may be several ways to remove the error trap:
-
Add pictures to improve operating procedures by showing the correct position or alignment for the valves or other equipment.
-
Replace some valves so they all operate the same way and make such changes using Management of Change (MOC).

Where poor lighting makes operations more difficult, recommend improving the lighting in the area to reduce errors and improve general safety. (Again, follow MOC)

Some companies consider error traps near-misses and want them reported using a near-miss or other reporting form.

Incorporate detailed assurance requirements of QA/QC for online leak sealing work including engineering design, fabrication, material selection and installation in the invitation to Bid(ITB)/contract document.

Improve the Online Leak Sealing Procedure by incorporating the specific assurance requirements for the QA/QC in engineering design, fabrication, materials delivery and installation.

revise the Emergency Response Procedure (ERP) to manage in the involvement of local authorities during the period of accident i.e. roles of Police Department.

-Started the day with Morning meeting at control room 8:30 am. There 1 safety and HSE moment. A helicopter ditching incident occurred at offshore Bintulu in between two platforms which resulted in one fatality involving PETRONAS staff. The incident happened when the helicopter encountered hydraulic system failure which escalated into fire hence resulted in the decision to conduct the controlled ditching into the sea.
-Presentation of contaminant in crude butanol at chemicals library.
